CONNECTING to COMMUNITY-BASED CARE

This section discusses transfer and coordination of care from NICU providers to community providers and the medical home.


IMPORTANT:

Families who have been in the NICU need support. Fortunately, there is a
community that is there, waiting to help.

Peer-to-peer support programs have been shown to be very effective and could be assets to every NICU.  After discharge from the NICU, many families would benefit from being connected to a community-based, peer-to-peer support program.

Connecting with other parents who have been in the NICU, understand what it's like, and can offer advice about how to navigate "life after the NICU" can be a lifeline.
